School-Based Health Clinics

Outreach Community Health Centers takes a client-centered approach by working to address all needs of individuals we serve. We offer services for primary care, behavioral health, and dental services providing school-based health clinical care within Carmen Schools of Science and Technology and Hmong American Peace Academy on the northside of Milwaukee.

The benefits of school-based service delivery for children and youth have been well documented. Among other indicators, availability of health services in school results in higher completion rates for immunization series among 12 to 18 year-olds than are achieved at other health center sites. For behavioral health, adolescents are ten times more likely to seek mental health support from a school-based center than through a community provider. This results in lower rates of depression and suicidal ideation.

Additional benefit and community services are available for families as needed through Outreach's Community Services providers.

Carmen Schools of Science & Technology

OCHC began our partnership with Carmen Schools of Science and Technology in 2023. Carmen Schools of Science and Technology is a non-selective public charter school consisting of three high schools, two middle schools and one elementary school servicing K4-12th grade students in Milwaukee.

Hmong American Peace Academy

Scholar enrollment has grown from 200 in 2004 to now serving over 1,800 scholars and is recognized as one of Milwaukee’s highest performing schools.
