Have you chosen someone you trust to make healthcare decisions for you in case of illness?
An advance directive describes, in writing, treatments you want or do not want. It tells others what to do with your care if you get injured and cannot express your wishes. You can complete an advance directive if you are 18 or older and of sound mind. Wisconsin Department of Health Services (DHS) has advance directive forms online. These forms are meant for you to complete without needing help from an attorney.
If interested in filling out paperwork that express your wishes, please come to a walk-in session or contact Kristin Simonson or Candace Coates for an appointment at (414) 727-6320. Appointments take first priority.
Appointments are open to patients and staff.

Holiday Wellness Fair
Holiday Annual Wellness Fair on Saturday, November 16, 2024, from 10:00 AM-2:00 PM. The Annual Holiday Wellness Fair's goal is to promote awareness of all aspects of wellness in our community of all backgrounds.
This free event will give health professionals an exciting opportunity to share educational information and provide valuable health and wellness resources through samples, health screenings, handouts and interactive activities. Open to the public.
Join the MCW Family Medicine Residency program participants as they screen for high blood sugar (Glucose), meet primary care providers and more!

Step Up Divas, a postmenopausal support group geared towards fitness and wellness. Our group’s monthly meetings focus on topics ranging from nutrition in menopause to improving your self-care.
Members will be able to participate in group fitness activities. Participants who come to two sessions are eligible for a fit bit.
