The clinic’s team of skilled dental professionals, including dentists, hygienists, and support staff, are dedicated to not only treating dental issues but also educating patients about preventive care and maintaining optimal oral hygiene.

Featured Programs
Prenatal & Child Care Coordination
Outreach Community Health Centers’ Prenatal Care Coordination and Child Care Coordination program is designed to provide supportive services and assistance to women at risk to experience adverse birth outcomes because they lack resources, have unstable housing, or experience high levels of stress and/or pressure. Child Health Care Coordination is provided for children up to age 7 to ensure children receive appropriate health services and scheduled immunizations.
Physicians provide routine outpatient prenatal and post-partum care, collaborating with our hospital partners who provide support during your delivery.
Hypertension
This program was built to assist providers in creating increased care management in Hypertension through assistance with increased education, increased encounters, and providing Bluetooth-enabled blood pressure monitors that store readings to the patients that join the program which assists providers in decision making for the patient in the future.
Diabetes
Certified Diabetic Education — Whether you were recently diagnosed or have had diabetes for a long time, Outreach’s Certified Diabetic Educator can help you develop a plan to stay healthy and give you the tools and support needed to make that plan a regular part of your life. Certified Diabetic Education is a recognized part of your care and is covered by Medicare and most health insurance plans.
Classes are led by our diabetes educator and registered dietitian. Food is provided and discussion topics vary depending on interest of group, but generally include nutrition, blood sugar management, and preventing complications.
Patient Centered Medical Home
A Patient-Centered Medical Home is a model of care that strengthens the clinical-patient relationship to ensure care coordination, whole person and long-term care. The PCMH puts patients at the forefront.
The PCMH model has been shown to help better manage patients’ chronic conditions, such as diabetes. PCMHs emphasize the use of health information technology and after-hours access to improve overall access to care when and where patients need it.
Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care. Outreach Community Health Centers gained PCMH recognition on August 31, 2023 from the National Committee for Quality Assurance.
All of Us Research Program
Outreach Community Health Centers is a proud partner of the All of Us Research Program, a nationwide effort to ensure diversity and inclusion in health research and to advance the practice of precision medicine for everyone. Outreach welcomes the All of Us team from the Medical College of Wisconsin into our health center to answer patients’ questions and assist with program enrollment.

School-Based & Satellite Services
Outreach Community Health Centers takes a client-centered approach by working to address all needs of individuals we serve. We offer services for primary care, behavioral health, and dental services to provide care to underserved in the north and northeast quadrants of the city where the school-based partnership was established. Approximately 17% of the adult population in this area remains uninsured.
OCHC began our partnership with Carmen Schools of Science and Technology in 2023. Carmen is a non-selective public charter school consisting of three high schools, two middle schools and one elementary school servicing K4-12th grade students in Milwaukee.

The Hmong American Peace Academy (HAPA) opened its doors in 2004 to 200 students, roughly 60 percent of whom were refugees that came to the United States in the last wave from Thailand, just prior to the opening of the school. Today, 15 years later, HAPA is home to more than 1,800 K4 - 12 students.
Family Medicine Residency Program
The mission of the MCW Family Medicine Residency Program is to train family physicians with the expertise and skills to provide individualized, evidence-based, culturally competent care to patients and families. This new program has been created through a partnership of Milwaukee’s North Side community health centers including Outreach Community Health Centers, the Medical College of Wisconsin’s Department of Family & Community Medicine, and Froedtert Hospital.
The program will address the growing need for compassionate and skilled family physicians who can provide outstanding care to underserved communities, directly addressing existing health inequities.
Community Services
Outreach Community Health Centers’ Community Services Division offers a range of programs designed to address the specific needs of individuals in our community, with services available to anyone, regardless of where they receive their health or behavioral care.
Our Comprehensive Crisis Management (CCM) and Targeted Case Management (TCM) programs provide personalized support to individuals facing significant behavioral health challenges or housing instability. These services, accessible through professional referrals, connect clients with a dedicated team of case managers, crisis stabilizers, mental health professionals, and peer support specialists. Our goal is to stabilize immediate concerns and guide clients toward long-term stability and connection to essential services.
The Benefits Acquisition team assists clients with navigating complex systems like Medicaid and SSI/SSDI, helping them access critical resources based on their individual eligibility.
For individuals experiencing homelessness, our Street Outreach program actively engages people in the community, connecting them to longer-term housing and services. The team seeks out individuals in need, building trust and helping them access support through collaborative partnerships, including housing programs and mental health services. Additionally, our PATH and Rapid Rehousing programs work to help individuals and families transition from homelessness to stable housing, providing ongoing case management, housing placement, and connections to vital community resources.
